I believe you would have to put this report in writing but you can add comments to your records.

Yes just type a letter of what she wrote wrong in your medical record and explain that you disagree what she put in your record type in the correct information it call ammendment it and don't forget send a copy of the medical records that's was wrong with the letter make sure you sign the letter the doctor will put it in your file when other doctors ask for your medical record.

Oregon I don't think they would remove it but like everyone else I would write it and add it to my paperwork noting that the statement is wrong.

A doctor can add an addendum to their dictation to change the original medical note. But good luck getting the doctor to admit that they were wrong. You can appeal the information in the report to the hospitals ethics committee. They will put pressure on the doctor. Worst case make a written complaint to the medical board in your state and that will get your doctors attention.

Is it a medical diagnosis or observation or is it a personality opinion type thing? If it's medical and you can prove its wrong as you said then I'm guessing that proof comes from another doctor and or testing in which case that info is already part of your record and the doctor that's wrong looks wrong to anyone reading your record anyway.

It was descriptive of "inappropriate affect, mood" "inappropriate for age" it totally makes me sound like I was out of control. In reality I was anxious... And she had her face in her laptop only glancing at me a couple times which added to my anxiety. I was in no way inappropriate in any manner! Hell, even when comp sent me to their IME doctors (3 times) each one said I was pleasant and cooperative. It's just upsetting to see her notes that came totally out of left field!😡

A lot of doctors that work for ssi or wkcop will always side on the company side ssi sent me to a doctor and he never looked at me nor gave me a physical exam how ever the report said I was fine and they also sent me a bill lol

The doc that made the half baked comments in my report was one I had made an appt with to establish care to be our family doc... Needless to say she's not even a contender now! Lol. I will be traveling further but I have an appt the 22nd with a different family practitioner to hopefully establish there.
