Medical Resume

I think I may have posted this before. I am not sure but with having some new people here. I thought it wouldn't hurt to post it again. I have created what I call a medical resume to take to my doctors because they are always wanting your medical information. So I designed a format to give that this information. I thought I would show you what I did just in case it will help someone who has a long medical history to share. Here it goes

This is the format. This has been very helpful for me and I have receives so many great comments from it from the doctors. They really appreciate it. It helps a lot with insurance companies to. I just thought I would share.

Very nice Profiler. I would also add a list of my medications/supplements - my format is this:

Medication Name - Dosage - Frequency - Time of Day Taken - Prescribing Doctor - What It's For - How Long You've Been Taking It (and perhaps any side effects that affect you)

I actually do this and I forgot to put that down. I am glad you put that in here. I was thinking it would be good for someone who had drug allergies to add them. I don't have any. But great thought. Thanks for putting it down. I forgot but so important to make sure it is in there.

I figured 😉. Yes! Allergies to drugs is so important, and even when you tell them you still have to watch them carefully cause I have had doctors forget and try to prescribe me drugs I am allergic to! So, very important indeed.
