Reading Doctor's Notes

I am looking at my doctor's notes and it gives several codes that I could enter on this app. However, in the notes it also says that the code item listed is being compared to another type of disease to see which one I have. I am unclear as to whether the code indicates I have that specific disease or is it just a list for a doctor to reference? Any help would be appreciated.

The medical codes list what the condition is next to it. I'd like to know if the medical code is a diagnosis or is something that could be placed on my file as something that needs to be researched and can be removed if they find that isn't what is wrong. I hope I am making sense. All of this has been a bit confusing.

The medical codes & descriptions are for documentation purposes.....the codes that are in your file will never be removed....its your medical medical record

You can look up all the codes on google. They are called ICD 10 codes and CPT codes. My coding books are old. I had to quit practicing in 2010, sorry.
